Various forms and documents of use to members of the Emeritus College are listed here. Most are available in both MS Word and Adobe PDF and some are interactive. Unless otherwise noted, forms may be downloaded, filled out and returned to the Emeritus College office in Old Main 102.

Member benefits forms

  • New!  Forms to apply for emeritus tuition waivers are now found on the Qualified Tuition Reduction Program page. Please use the form for ASU affiliates. Members of the college who participate substantively in a conference or other scholarly meeting (delivering a paper, chairing a session, participating on a panel, attending a committee meeting as a committee member, etc.) may apply for a travel assistance grant of up to $200.
