Forms and Applications
The following are links to various forms and documents of use to members of the Emeritus College. Most are not interactive, but are available in both MS Word and Adobe PDF. Unless otherwise noted, forms may be downloaded, filled out and returned to the Emeritus College office, Old Main 102.
Retired faculty who have received official emeritus status from their college or university, which has similar accreditation standards to ASU, and who reside in the greater Phoenix area for all or part of the year, may apply for Associate Membership. Associate Members enjoy most, but not all, membership benefits available to Regular Members.
For questions about emeritus tuition waivers, please contact Elizabeth King at email@example.com or 480/965-7405. Forms can be emailed as PDF attachments to firstname.lastname@example.org. Forms can also be faxed to 480/965-0785, but please call or email to give a heads up that a fax is coming.
Members of the College who participate substantively in a conference or other scholarly meeting (delivering a paper, chairing a session, participating on a panel, attending a committee meeting as a committee member, etc.) may apply for a travel assistance grant of up to $200.
